Connecting Need to Resource

In August 2011, PPACG established a steering team and contracted with Jody Alyn Consulting to develop a strategic plan for a system of care collaborative to better serve the military community of the Pikes Peak region. This strategic planning process resulted in the completion of an assessment report and strategic plan that guide implementation of the Peak Military Care Network.

OUR MISSION
The Peak Military Care Network (PMCN) was established to ensure that all former and current members of the military and their families receive the highest standard of care for their behavioral health, social services and community integration needs. Our goal is to eliminate gaps in support, reduce duplication of services, and significantly improve the efficiency and effectiveness of the network through increased collaboration.

OUR PRIORITIES

PMCN intends to create a model that can be adapted and replicated in other communities. PMCN’s first actions will be to: 1) build and vet the network of service providers, 2) develop a “navigation  collaborative” to improve access for all service users, and 3) provide tools to advance the network from stand-alone organizations to an integrated service system. Core values, strategies, and goals are set forth in a strategic plan.

OUR WORK

  • PMCN will serve as a central source for information, resources, and integrated services under the initial direction of PPACG.
  • The initiative is an outgrowth of the Fort Carson Regional Growth Plan, a comprehensive planning effort that began in 2006 to anticipate the effects of rapid military growth at Fort Carson.
  • In May 2011, PPACG established the Network of Care website to provide a centralized point of entry for information and resources for the military community. The site includes a searchable service directory, a library of health- and veteran-related articles, community announcements, employment information, legislative information, and care coordination features.
  • The PMCN Assessment Report is a comprehensive report that examines the challenges confronting the users of the region’s military system of care, as well as solutions, including insights from an unprecedented gathering of 105 participants in 12 focus groups, including government, military and business leaders, first responders, crisis intervention service providers, educators, nonprofit organizations, and military users of the system, that provided a powerful snapshot of a system challenged by high demand and dwindling resources.
  • PMCN will be formally launched in the spring of 2012.
  • PMCN’s Steering Team is a volunteer body responsible for oversight of the strategic planning process and includes representatives from 17 entities in the region, including local military installations, community and business leaders, and local governments as follows:
